Serafim Hospice, Inc. provides a full set of healthcare services developed to meet the special needs of patients in times of crisis due to terminal illness. We are here to reduce anxieties for families and make difficult times graceful for patients on their end-of-life journey.

Our Team

Nurses

Skilled Nurses

Evaluate and assess the condition of the patient during the visits. Specifically trained to manage pain and symptoms, coordinate the treatment plan, and educate and instruct the family in correct patient care and drug administration. Available 24 hours a day, 7 days a week.

Medical Director

Certifies the prognosis given by the attending physician. Provides the key connection and participation at each stage of the patient's ongoing care. Organizes and manages the hospice program's administrative tasks as well as the overall local operations. Accountable for the daily administration and running of the hospice services.

Social Worker

Social Worker

Assists patients and their families with planning for end-of-life care, understanding their treatment plan and speaking up for their needs. Managing the emotional, familial, and financial strains associated with life-limiting physical illnesses, resolving crisis situations, and connecting to other support systems. Provides counseling/bereavement services and community referrals.

Homecare

Certified Home Health
Aides & Homemakers

Provide and assist with personal care needs. Responsible for observing the patient, reporting these observations and documenting observations and care performed.

Chaplains

Chaplain

Develops, documents and delivers a hospice spiritual care plan. Provides direct spiritual care and competently examines, coordinates, and evaluates the spiritual needs of hospice patients and their families.

Volunteers

Available to provide companionship and assistance through visits, legacy creation, reading, and shopping.  Provides respite care for the primary caregiver to have an opportunity to take a step away and reenergize.

Who pays for hospice care?

Hospice care is paid by medicare, health insurances, hmo’s, private pay and community support such as donations and volunteering.


Where are services provided?

Hospice services are provided wherever the patient is residing and calls home. This includes patients' home, skilled nursing facilities, assisted living centers, etc.
